Lorain County Health & Dentistry (LCH&D) has been recognized by the Health Resources and Services Administration (HRSA) with multiple Community Health Quality Recognition (CHQR) badges for its outstanding performance during the 2025 reporting year.
The annual CHQR badges recognize Community Health Centers across the nation that demonstrate exceptional clinical quality, improved patient outcomes, expanded access to care, and excellence in using health information technology to serve their communities.
Among the badges awarded to LCH&D are:
- National Quality Leader – Diabetes Health
- Preventive Health
- National Quality Leader – Heart Health
- Health Center Quality Leader – Silver Awardee
- Advancing Health Information Technology for Quality
These national recognitions highlight LCH&D’s continued commitment to providing high-quality, accessible, and patient-centered healthcare while improving health outcomes across the communities it serves. The Community Health Quality Recognition program highlights health centers that consistently meet or exceed national benchmarks in preventive care, chronic disease management, health outcomes, quality improvement, and the strategic use of health information technology. The awards are based on performance data submitted annually through HRSA’s Uniform Data System.
In 2025, Lorain County Health & Dentistry provided comprehensive healthcare services to more than 17,500 patients through more than 57,000 visits across its six locations in Lorain, Elyria, Oberlin, and North Ridgeville.
